I agree to the terms and conditions of LA Cheerz Withdrawal Notice:

Payment will continue until 2 weeks after the withdrawal notice regardless of attendance or participation. I understand full tuition is due until 2 weeks after withdrawal request form is turned in. Tuition is due on the first of every month; therefore unless a request is submitted by the 15 of the previous month, I will be charged for the month.
