Has your athlete ever cheered competitively? If so, what program/level? *Examples: LA Cheerz Youth Novice; Mini Prep 1.1; Junior 2 Elite, etc.

What stunting level has your athlete mastered? *This refers to the highest stunt level your athlete has competed in a routine and the position they performed (flyer, base, or backspot). If your athlete has competed at multiple levels or positions, you may select all that apply. (Examples: Level 1 Flyer or Level 3 Base & Level 3 Backspot or Level 2 Flyer & Level 3 Base))

What tumbling level has your athlete mastered? *Mastering a tumbling level means that your athlete is able to throw that skill successfully on the mat without needing assistance. EX: If your athlete has mastered their back handspring and are currently working on mastering tucks, then you would select Level 2.
